The main difference is the dining experience. The First Class lounge has a fine dining option where you can order extensively a la carte. This is restaurant-style table service. The Business Class lounge does not have this. There is a buffet option in both lounges but the difference in the quality of the food is quite noticeable.

The second biggest difference I've found is the tranquility factor. Both lounges are similar in their furnishings and upholstery, however, the Business Class lounge is generally quite busy. The First Class lounge on the other hand is not. There are more Business customers and Gold frequent flyers than there are First customers and Platinum frequent flyers, and so because the lounges are the same size, the First Lounge is inevitably much more peaceful.

The only other differences I've noticed are that the bathrooms and amenities are slightly nicer and shinier in the First Class lounge. Not by much though and this is to be expected.

All up the Business Class lounge is so good compared to other Business Class lounges around the world that I'd put it somewhere between a regular Business and First lounge. You won't go wrong with either.
